Rating Card - 2007- Varsity Crew to rate all non varsity Game
Situation Report (word)- to submit problems, situations, questions Liability Alert Form
(excel)- should be completed when injured player is removed from game site by
ambulance or Official thinks injury will require off site Medical attention. Disqualification/
Incident Report (excel)- required when a Player / Coach has been
disqualified from participating in a game or any incident occurs. Doctor Permission Form - to be used by a Physician to allow a player to participate who was removed from a game because Officials determined the player was unconscious or apparently unconscious. (excel) Referee
Application - to apply for the white hat. Return to Referee Committee. (excel
